Job description
We are seeking a Family Medicine Physician to join our healthcare team. The Family Medicine Physician will provide comprehensive medical care to patients of all ages in a rural health clinic setting. Responsibilities include direct patient care in the outpatient rural health clinic as well as on-call obligations for the critical access hospital with a long-term commitment to full-time pediatric hospital call. The physician will participate in quality assurance programs, peer review processes, and maintain compliance with regulatory requirements. The position also entails involvement in administrative tasks, utilization review activities, and attendance at clinic, department, and hospital meetings as required by Livingston HealthCare Medical Staff Bylaws.

ESSENTIAL FUNCTIONS, DUTIES, AND RESPONSIBILITIES:

Provide comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and seniors. This encompasses preventive care, routine check-ups, and management of acute and chronic medical conditions. Clinic requires an Internal Medicine focus.

On-call Critical Access Hospital pediatric coverage typically includes ~eight, 24-hour shifts per month managing newborn resuscitation, pediatric critical illness stabilization and transfer, hospital admission with comprehensive inpatient care and coverage of the, "Kids Care Line," that answers family phone calls from the clinic.

Monitor patients' health status, adjust treatment plans as necessary, and provide ongoing support to promote disease control and quality of life.

Skilled in managing chronic conditions in older adults, including diabetes, heart failure, and post-cancer treatment surveillance, with a focus on the needs of an aging population.

Committed to welcoming and transitioning patients into the primary care medical home following an acute hospitalization or emergency department visit.

Offer patient-centered education and counseling on various health-related topics, empowering patients to make informed decisions about their healthcare.

Manage care transitions, communicate effectively with other members of the healthcare team, and advocate for continuity of care to optimize patient outcomes.

Participate actively in Livingston HealthCare’s quality assurance programs to maintain and enhance the standard of care.

Engage in the peer review process to contribute to ongoing professional development and quality improvement initiatives.

Maintain compliance with annual education requirements and regulatory standards.

Complete all required administrative tasks, ensuring accurate and timely documentation of provided services for billing purposes.

QUALIFICATIONS (Required):

Valid state medical license to practice in Montana or ability to obtain licensure

Medical degree (MD or DO) from an accredited institution.

Board certification or eligibility in Family Medicine.

DEA registration and other necessary certifications for practice.

Strong commitment to patient-centered care and community health.

Excellent communication, community leadership and interpersonal skills.

Ability to work collaboratively in a multidisciplinary team environment.

Experience in rural healthcare settings preferred.
