Locums – Fortuna, California, Primary Care Job Clinical Role:

    • Start Date: 7/15/2024
    • Ongoing needed
    • Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted fo
    • Includes some Chronic Pain Management
    • Chart closure within 72 hours
    • Schedule = 18 PPD, minimum see 18 PPD
    • Monday – Friday (8 – 5)
    • A mix of peds/adults (Will consider IM only)
    • A decision is typically made within 7-14 days
    • We will also accept DO
    • BE will be considered if truly BE (5 years or less since residency)
    • Active DEA needed before the start
    • Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely)
    • 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years)
    • Primary series plus booster, regardless of the date received or 1 dose of the current COVID vaccine (2023/2024 formulation) – Required
    • HCO & Vendor agree to a 30-day notice for ending all assignments
    • Patients Per Shift: 16
    • Coverage Type: No Call
    • EMR System: Epic
    • Board Certified – Required
    • BLS – Required
    • Active CA Liscense – Required
