- Work Dates Needed: ASAP - Flexible 4 day work week (see below for clinic hours), call is 1 week/mo
- Hours: M-F 8-5; Sat 8-11 (closed for lunch 12-1)
- Preferred Schedule: Flexible 4 day work week (see above for clinic hours), call is 1 week/mo.
- Worksite Setting: Outpatient work site, FQHC clinic prenatal, OB & Peds to Geri
- Scope of Work: MD sees 2 - 3pph, NP sees 2pph / 12-16 patients/shift
- Licenses, Certifications, Requirements: BC FM w/OB exp. but open to OBGYN, BLS, DEA, IN State Licensure
- EMR: Allscripts (Scribe)
- Credentialing: 1-3 months
