The Physician Lead serves as a health-care professional and capable of handling a variety of health-related problems. The Physician Lead works on problems of diverse scope and complexity ranging from moderate to substantial.

Work Schedule: Full-time/40 Hours per Week

Job Type: Physician Lead, Hybrid

This position is considered a hybrid position. This position will provide direct patient care 3-days per week and require driving/travel to the assigned territory.

The assigned territory will service parts of Atlanta, GA.

Additionally, this role will require performing supervisory and administrative functions, 2-days per week at home.

This position is considered full-time, Monday through Friday.

The Physician Lead serves as the leader for a team of up to 10 advanced practice providers who provide home-based primary care to our senior population. In addition to supporting our advanced practice providers, you provide in-home and telemedicine visits to high-acuity seniors 3-days a week. The Physician Lead work on problems of diverse scope and complexity ranging from moderate to substantial.

The Physician Lead focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgement and decision making on complex issues regarding job duties and related tasks and works under minimal supervision. Uses independent judgement requiring analysis of variable factors and determining the best course of action.

  • Provide direction and oversight to a team of up to 10 advanced practice providers.
  • Provide collaborating agreements for up to 4 nurse practitioners in the state of GA.
  • Consults with advance practice providers on management of complex patients.
  • Lead daily huddles as part of our interdisciplinary teams.
  • Review all acute utilization events, with goal of reducing unnecessary utilization.
  • Will see patients 3-days per week directly in-home and via telehealth, with goals set to drive interpersonal relationships with patients and families.
  • Use of laptops for EMR charting.
  • As needed/ongoing attention to prescription refills, home health referrals/co-signatures, lab follow-ups and coordinated care management.

Required Qualifications

  • Graduate of accredited MD or DO program of accredited university.
  • Experience leading a team of 10+ advanced practice providers.
  • Experience providing collaborating agreements for up to 4 advance practice providers.
  • 8 or more years of technical experience.
  • 2 or more years of project leadership experience.
  • Must have an active/unrestricted state license, DEA license, and board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Current state driver's license and proof of auto insurance.
  • Senior patient care experience.
  • Active BLS through the AHA (American Heart Association) or ARC (American Red Cross).
  • Strong technical experience.
  • This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
