Family Practice physician opening in SE Indiana, within 1 hour to Louisville, Cincinnati and 45 min to Indianapolis

# of Non-Clinical Support Staff: 6 # of Clinical Support Staff: 12 (MA’s, Rad Tech, Lead MA)

Physician will work with his/her own RN/LPN/MA

# of Patient Rooms: 20 - new building in 2014

EMR: EPIC – CRH/Practice

On-Site Ancillary Services: 2 Quality Care Nurse/RN & QMA (RetinaVue); Lab (draws only) & X-Ray

- Practice Day average patient volume is 18

Types of Outpatient Visits: Acute Office Visits, Physicals, New Patient Visits, Annual Wellness Visits, Medicare Wellness Visits, Follow-up Established Visits, and Acupuncture (Watanabe)

No inpatient rounding. They partner with a Hospitalist group

Office Hours & Call Schedule:

Practice Operating Hours: Monday – Friday 8-5pm

Anticipated Work Schedule for Recruited Physician: 4 days a week (8-5pm) for appointments & 1 day a week for administrative duties.

Call Schedule Details (include frequency, on call duties): On call 1 week every 5th week (continuous rotation). On call consists of taking calls after clinic hours & during the day even if on clinic to cover for needs when other doctor is out of the office, & calls from incoming physicians concerning patients.

Candidate Criteria:

Visa Support: H1B, not a J1 waiver visa qualifying location

The hospital has been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program.

Compensation Information:

  • Guarantee Period is the first two years of the contract
    • Guaranteed compensation of $260,000/year
    • Paid as a) $205,000 annual salary, plus b) $13,750 per calendar quarter
  • After Guarantee Period
    • Base salary of $205,000/year
    • Plus production incentive of $52.42/wRVU for wRVUs above base salary
    • If production incentive is paid, and metrics are met, an additional $5.82/wRVU of
      the value-based incentive will be paid (percentage of metrics achieved will be a percentage of incentive paid)
    • Production and value-based incentives are calculated and paid by calendar quarter
  • CME allowance is $4,000 annually throughout the contract (add $1,000 for each additional board-certified specialty, up to $8,000/year)
  • Initial recruitment incentive of $100,000, tied to a 4-year commitment
  • Provider Paid Time Off (PPTO) is an accrual model and amounts are as follows:
  • Accruing .15 PPTO hours per paid hour (39 days for full-time
    • PPTO days are to be used for vacation, sick days, CME time, bereavement, holidays, and other practice closures, etc.
    • Unused PPTO hours roll over to subsequent years
