**Locums Dental opening in Fortuna, CA**
* Dates: 5/13/24 - ongoing
* Must be willing to work 3 months minimum with the option to extend
* Rates: let us know!
* Outpatient only
* Schedule: Monday - Friday, 8am - 5pm
* 50% Adults/50% Kids
* EMR: Epic
* Allowed holidays: New Year's Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, & Christmas Day
**Requirements**
* CA License
* Active DEA before start
* Clean history
* BLS
* Immunizations: 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Primary series plus booster, regardless of the date received or 1 dose of the current COVID vaccine (2023/2024 formulation)
