We are seeking Primary Care Physicians to join us on our journey in Connecticut. As a Primary Care Physician, you will be responsible for managing patient care and treatment in collaboration with other team members. Your role will involve providing exceptional patient care, performing health assessments, and actively managing common and complex conditions. You will also be responsible for ordering and interpreting diagnostic tests, formulating care plans, and explaining medical procedures and plans of care to patients and their families. Additionally, you will prescribe medication or other forms of treatment as necessary.

Responsibilities:

  • Provide exceptional patient care, ensuring a high standard of medical service
  • Perform health assessments and screen for preventative health measures
  • Actively manage common and complex conditions, adapting treatment plans as necessary
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and healthcare needs
  • Formulate and document care plans based on thorough assessments and medical knowledge
  • Explain medical procedures and plans of care to patients and their families, ensuring they understand the treatment process
  • Prescribe medication or other forms of treatment according to patient needs and medical guidelines
  • Participate in an on-call rotation as needed to provide continuous patient care
  • Communicate assessment findings and collaborate with the healthcare team to identify gaps in care and improve patient outcomes

Qualifications:

  • Board eligible or board certification in Internal Medicine, Family Medicine, or Geriatrics
  • Current CT
  • DEA license (or eligibility for registration)
  • Demonstrated interest and passion for value-based care delivery models
  • Strong oral and written communication skills to effectively interact with patients, families, and the healthcare team
