Requirements
- Board Certified
- BE will be considered if truly BE (5 years or less since residency)
- Active DEA
- Active CA License
- BLS
- The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years.
- Covid vaccine; primary series plus booster, regardless of date received OR 1 dose of the current COVID vaccine (2023/2024 formulation)
