Primary Care Physician Position Details:

Hartford HealthCare is seeking a full-time Family Medicine or Med-Ped Physician in central Connecticut.

Role: Primary Care Physician

Location: South Windsor, CT

Setting: Primary Care (birth – geri, no OB)

Schedule: Full-time, weekdays only, ask about a 4-day work week option!

Call: Infrequent phone call for work/life balance

Primary Care Innovation at Hartford HealthCare:

All healthcare systems need to tackle the challenge of patient access in Primary Care, but only the most innovative systems look beyond ‘more patients per day’ and ‘work harder, longer’. Hartford HealthCare recently partnered with a MIT-trained data scientist to better understand each of our 350,000 patients; to look beyond numbers and further into the complexity of each patient’s needs. Through this holistic analysis, functional updates were made to:

• Group our nearby primary care practices into small teams, so if one provider is booked solid, another might be available to see a patient while maintaining continuity of care
• Prioritize patient visits by appointment type, so patients with urgent needs aren’t waiting too long to be seen
• Adjust the suggested frequency of preventative exams based on age, prioritizing older adults with complex health issues over young, healthy patients
• Expand our virtual health team to support offices across the network, ensuring more opportunities for immediate patient appointments
• Offer a specialized team for Medicare Annual Wellness Visits, freeing-up providers’ time to see patients for follow-up visits or chronic care management

These are only the first steps though. We will continue to leverage advanced data analytics and machine learning algorithms to predict trends in patients’ and provider needs.
