Los Angeles General Medical Center is the cornerstone of the safety-net for the largest county in the United States. As one of the largest public hospitals in the US, we are home to one of the largest GME training centers in the US, with more than 950 residents and fellows, representing nearly 1% of all post graduate trainees in the US. The PCMH Physician provides direct care to, and assumes responsibility for, all patients in their panel. They also may supervise trainees providing direct care to patients within the PCMH, providing high quality teaching and clinical care. In addition, they oversee clinical panel management provided by the entire PCMH team and collaborating with and supporting the Nurse Care Manager in the care of high-risk patients. The physician works with all team members to achieve and maintain desired patient outcomes in a cost-effective manner. The physician plays a central role in the development, implementation, and evaluation of shared care plans for individual patients and groups of patients in collaboration with other PCMH team members. The physician may receive other primary care priority assignments including operations, population health, integrated behavioral health or perform backup provider duties as indicated for patient care/safety needs.

About the Division of Primary Care
Los Angeles General Medical Center is one of the largest sites within the Department of Health Services providing essential primary care to patients from across the greater Los Angeles County and a recognized leader in social and behavioral health integration to meet many safety-net community needs.

Services are provided in a safe, patient-centered environment through a network of 8 medical home clinics that deliver primary care for both adults and children. Los Angeles General Medical Center practices in a patient-centered medical home model and patients are provided services by the same team of healthcare providers to support all their health needs. The medical home serves as a hub for the larger network of hospitals, emergency departments, urgent care centers, behavioral health providers, and specialists within the Los Angeles County Department of Health Services.

Qualifications

Position Requirements: 
• Candidates must have an MD, DO or equivalent degree
• Candidates must be board-certified or board eligible in Family Medicine, Internal Medicine, or Internal Medicine/Pediatrics
• Candidates must have or be eligible for a California medical license or equivalent certification/permit as determined by the Medical Board of California
• Desire to practice at the nations’ largest teaching hospitals at a single site, which is conveniently located on a medical campus adjacent to downtown Los Angeles
• Passion for providing high quality clinical patient care to an underserved population
• Passion to teach and work with fellows, residents, and medical students
• Opportunities to participate in multidisciplinary conferences, quality initiatives, and hospital committees



Clinical
• Provides direct patient care
• Provides supervisory patient care and support for physician trainees
• Clearly documents all patient encounters in the electronic record on the day of the visit
• Reviews and acts upon diagnostic test results as appropriate; ensures patients are aware of test results; documents acknowledgement of results and actions taken in patients’ ORCHID record
• Responds within 3 business days to inbox messages, proposed orders, and notifications from other DHS staff; some orders may require a prompter response (e.g., medication renewals)
• Communicates with specialists as appropriate via eConsult. As clinically appropriate, carries out specialists’ recommendations and closes eConsults in a timely fashion
• Through regular communication, assists the Care Manager in prioritizing high-risk patients, understanding factors which contribute to their high-risk assignment, and the provision of complex care management
• Leads multidisciplinary family conferences for high-risk, complex cases as needed
• Closes care gaps using ORCHID’s Health Maintenance tab or Registries widget
• Communicates patient care priorities, especially follow-up plans that involve non-providers (e.g., high-risk patients that need Care Manager follow-up) to medical home team members
• Teaches on topics of adult primary care for trainees during supervisory clinic sessions and ad hoc
• Reviews, provides feedback, and signs off on resident charts that were staffed during supervisory sessions
• Leads teaching sessions for clinic staff
• ELM: Reviews ELM Scorecard and initiates team actions (e.g., outreach) to close Care Gaps and improve chronic disease management for empaneled patients based on registry performance; communicates patient management priorities to PCMH team based on ELM Top Opportunities and guidance from clinical leadership; makes final decisions about satisfying clinical measures in ORCHID; determines when an ELM registry or measure does not apply to a patient (requests Exclusion); requests Unattribution for patients with incorrect empanelment; and alerts the Enterprise Help Desk when data integrity problems are detected
• Leads/participates in team huddle (AKA Daily Brief) before the first clinical session of the day to review day-of-care plan for scheduled patients
• Serves as expert clinical resource for medical home team, including urgent and non-urgent situations
• Refers patients to appropriate resources as indicated by screening for social and behavioral determinants of health (SBDOH)
• Responds to Code Assist calls within the facility as assigned

Administrative/Leadership
• Participates in PCMH and multi-disciplinary team meetings
• Communicates PCMH team issues or needs to leadership as appropriate
• Participates in performance/quality improvement (PI/QI) activities
• Responds to patient complaints and grievances in a timely fashion
