Job Description:

ClearMD is patient focused and our providers can take special satisfaction in the knowledge that we allow them the time and encouragement to build relationships with their patients. Our PCPs provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care and health maintenance to patients. They serve as leaders within the patient centered medical home model and work collaboratively with the care team to ensure that each patient gets the best possible care.

Responsibilities:

  • Complete initial history and physical assessment, and evaluation of new patients
  • Complete semi-annual and annual assessment of patients
  • Complete reassessment of established patient after a significant change in the patient's health status
  • Consults and refers with/to appropriate “Specialty Physicians”
  • Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
  • Ensures documentation is recorded in a complete, timely and orderly manner, utilizing templates in the electronic medical record.
  • Provide ongoing monitoring of patient's medical condition and provides treatment
  • Order prescription medications and diagnostic procedures
  • Authorize and coordinates all specialty and ancillary services
  • Participate in their patients' interdisciplinary team case conferences to define problems and work as a member of the team to build a goal oriented care plan
  • Provide high quality, evidence-based out-patient care for illness & injury for psychosocial & preventive health needs consistent with the full scope of services
  • Contribute to QI, peer review & national accreditation efforts in-service training, support service enhancement, special projects, and committee work.

Education and Experience Requirements:

  • A Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree is required.
  • Three years post graduate experience is preferred, but not required.
  • Must have an active medical license in New York.
  • Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, Critical Care preferred.
