• Setting: 2 days in clinic and 3 days in skilled nursing home.
• Requirements: VT or IMLC licensing. BC in IM, FM, Geri
• Procedures: Preventative visits, acute needs, and chronic ongoing needs. SNF admissions, discharges, acute needs, and periodic assessments.
• Pay: $2,400/day + Travel and Lodging
