Closest Airport/Distance to Assignment Location: Bloomington-Normal (BMI / KBMI). 34 miles from the center of Fairbury, IL.
JOB DETAILS:
· Practice Type: Standard family medicine clinic shifts
· Shift hours: 8a - 5p
· Call required?/Rotation or Schedule: N/A
· Schedule: Monday – Tuesday - Wednesday
· Specific Dates Open: 4/3 - 4/28 - with opportunity to extend
- Types of Cases: Standard FM patients
- Patient population/caseload:
- 15-18 patients per day
- All ages

- Staff on Shift: Full time NP and additional MD works part time Tuesday/Wednesday/Thursday
- EMR: EPIC
- Credentialing: 45-60 days
- Licensing: Active IL License
- Required Certifications/Cred Items: BC, BE within the last 12 months of completing residency | ACLS, BLS
- Minimum # years’ experience: 1
