- Option to work 4 or 5 days
- This is a full-time physician role caring for Medicare and Medicaid geriatric patients in the home setting.
- Our goal is to see patients with realistic time, to provide the right level of care to make the clinical impacts outlined below:
- The Physician Team Leader (Pod Leader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team.
- Success in Value Based Care changes the demands placed on primary care. We are investing in physicians to do what they do best: provide outstanding care to complex patients with a complete care team.

- Board Certified Family Medicine, Internal Medicine or Geriatric Medicine
- Active and clean state license
- DEA clean and active
- Have Medicare and Medicaid numbers
- Geriatric experience a plus
