Seeking a Board Certified Family Practice physician for this Federally Qualified Community Health Center for a full time physician at this growing, multi-service practice. The practice implements a Patient Centered Medical Home model and has integrated oral and behavioral health care along with case management. Providers are assigned to a team consisting of an RN, medical assistant, social worker and patient navigator to support the patients through all aspects of their care.
Practice has a fully integrated EMR through all disciplines.
Experience with underserved communities is a plus. Candidates must be passionate about increasing access to preventative and primary healthcare. Consisting of three locations and offering a comprehensive approach to management, this group focuses on the management of healthcare and individual vs. community needs. Opportunities exist for special interests within communities.
Candidates should be community oriented and skills should include a strong clinical background in Family Medicine, support and oversight of mid-levels, and strong communication skills. Salary is $160k and benefits include five weeks of paid time off, 12 paid holidays, FSA, pension, CME time and stipend, licensing costs covered, and health/dental/malpractice insurance.
