Job Details:

• Practice Setting: Primary Care practice
• Coverage: 3 days per week beginning May 15
• Average Number of Patients Per Day: 22
• Number of Physicians in Practice: 0
• Will Other Internal Medicine Physicians Provide Coverage During Locums Coverage Dates: No
• How Many Mid-Levels Provide Coverage: 1
• Will the Locum be Responsible for Supervising Other Health Care Providers: No
• Will the Locum Tenens Physician Respond to In-House Codes: No
• Will the Locum Tenens Physician be Expected to Provide Backup for the ED: No
• Number of Exam Rooms: 3 assigned to provider total of 10 in practice
• Average number of OB patients Per Day: 0
• Services Available On-Site: Laboratory, Radiology
• Documentation System/EMR: Epic

• Emergency Supplies Available: Defibrillator, Airway
• Practice Population: Adult, Geriatric, Pediatric

Required General Procedures:

• Required Medicine Procedures: Adult Outpatient, Without ICU/CCU
• Required Pediatric Procedures: General Outpatient
• Required Gynecology Procedures: Pelvic Exam / Pap Smear
• Required Surgical Procedures: Suturing of Minor Lacerations, I & D (Incision & Drainage)
• Estimated Credentialing Timeframe: 45-60 days

Submission Requirements

• Board Certified/Board Eligible - [REQUIRED]
