**Geriatric Medicine opening in Wisconsin**
* Located in ***Shawano, WI*** - just 30 miles from Green Bay & 107 miles from Milwaukee!
* Full-time, permanent opening
* Hospital employed position
* Provide care to Skilled Nursing and Long-Term Care facilities
* Enjoy a flexible daily schedule and work with a team that is genuine, customer focused, and has a passion for geriatrics!
* See patients in their own environment
* Hours: 7am - 5pm
* Call: 1:8 (phone calls only do not go into facilities when on call)
* Monday-Thursday, 5pm-7am
* Friday, Saturday, Sunday 24 hours
* Nurses triage calls during office hours, prep documents, and prioritize work load for provider; scheduler completes scheduling and administrative tasks
* 10-12 visits per day
* Mobile and electronic platform; labs and x-rays quickly available
* Chronic disease management and population health management
* Coordinated visits allow for flexible schedule (start/end time)
* Care setting allows you to get to know your patients
* Involvement in quality and process improvement is encouraged and supported
