Job Details
- Shifts include: Monday - Friday 8:00a - 4:30pp, On average, there are seven (7) on-call days (evenings/weekends) with weekend telephone triage.
- Duties entail: annual assessments, Post-hospital visits, Acute visits
- Minimum 1 year working with the frail or elderly population
- MA license in hand - Required
