Home Based Primary Care Physician Opening- Lexington, KY

We are seeking a Home-Based Primary Care Physician to provide comprehensive medical care to patients in the comfort of their homes, assisted living facilities, and personal care facilities. This is an opportunity to provide relationship-based primary care to patients who may have difficulty accessing traditional office-based medical services.

In addition to providing direct patient care, the physician will serve as a clinical resource, supervisor, and subject matter expert for nurse practitioners providing home-based primary care in the same settings.

Position Highlights

• Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

• Call: Occasional weekend and weekday after-hours call

• Provide care in patients’ homes, assisted living facilities, and personal care facilities

• Clinical leadership and support for home-based primary care nurse practitioners

Responsibilities

• Provide comprehensive primary care to patients in their homes, assisted living facilities, and personal care facilities.

• Evaluate, diagnose, and treat chronic medical conditions, as well as acute and urgent illnesses and injuries.

• Develop and manage individualized treatment plans in collaboration with patients, families, caregivers, and the healthcare team.

• Order, review, and interpret appropriate laboratory and diagnostic studies.

• Prescribe and manage medications, including controlled substances as appropriate.

• Perform procedures within the physician's scope of practice, established protocols, and available equipment.

• Make referrals to specialists and coordinate care when additional evaluation or treatment is needed.

• Provide clinical supervision, guidance, and support to nurse practitioners practicing in the home-based primary care setting.

• Serve as a clinical resource and subject matter expert for the home-based primary care team.

• Collaborate with interdisciplinary team members to promote continuity of care, quality, safety, and positive patient outcomes.

Qualifications

• M.D. or D.O. degree required.

• Current Kentucky medical license, or eligibility for Kentucky licensure.

• Current DEA registration and controlled substance authority required.

• Board certified or board eligible in an appropriate specialty; board certification expected within five years of employment.

• Fellowship training in Geriatrics is preferred but not required.

• Valid driver's license and reliable personal transportation required.

• Automobile must be maintained in good working condition and insured in accordance with Kentucky requirements.
