This is a full-time, 40-hour-per-week opportunity for an experienced Nurse Practitioner who enjoys variety, team-based care, and serving diverse patient populations in a community health setting.

The ideal candidate will be comfortable delivering high-quality, patient-centered primary care while collaborating with physicians, advanced practice providers, nurses, medical assistants, and support staff.

Job Responsibilities
Assess, diagnose, treat, and manage acute and chronic medical conditions
Perform preventive care, wellness visits, health screenings, and patient education
Manage chronic conditions such as diabetes, hypertension, asthma, COPD, hyperlipidemia, and related primary care concerns
Collaborate with physicians, APPs, nurses, medical assistants, and care team members
Maintain accurate and timely documentation in the electronic medical record
Support continuity of care and assist with clinic coverage needs as assigned
Deliver culturally responsive, compassionate care to patients from diverse backgrounds
Qualifications
• Family Nurse Practitioner certification preferred
• Current DEA license or eligibility to obtain
• BLS certification
• Experience in family medicine, primary care, community health, or FQHC setting preferred
• Spanish-speaking ability strongly preferred
• Strong communication, documentation, and clinical decision-making skills
• Ability to work effectively in a collaborative, fast-paced outpatient setting
• Flexibility to support multiple clinic locations as a float provider
