Physician/Doctor - Family Practice
Location: IN
Position Overview
Outpatient medicine focused on continuity of care, health maintenance, and disease prevention
Evaluate and manage moderately complex to complex health-related problems
Maintain medical histories and accurate medical records
Refer patients to specialists as clinically appropriate
Required Qualifications
Graduate of an accredited MD or DO program
Active licensure meeting the requirements of the state of jurisdiction
Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
5 or more years of technical experience (as documented)
Credentialing / Onboarding Items
This is a patient-facing role and requires Tuberculosis (TB) screening if selected
