Baptist AgeWell Center for Health is seeking a compassionate and experienced Geriatric Physician to join our growing senior-focused primary care team in Clay County, Florida. This is an exceptional opportunity for a physician who is passionate about caring for older adults and helping patients maintain their independence, health, and quality of life.
As part of Northeast Florida's most preferred health system, you'll practice in a collaborative, physician-led environment with extensive support services that allow you to focus on delivering exceptional patient care.
Position Highlights
- Full-time, outpatient senior primary care practice
- Monday through Friday schedule, 8:00 AM - 5:00 PM
- Minimal call responsibilities
- Lead a collaborative care team alongside experienced Advanced Practice Providers
- Epic EMR
- Physician-led, locally governed organization that values physician autonomy

Qualifications
- MD or DO
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active or eligible Florida medical license
- Passion for senior care and population health
- Minimum of 5 years of experience in Internal Medicine, Family Medicine, Hospital Medicine, or related practice with senior patients
- Geriatrics experience preferred
