Overview:
Our geriatrics group on Long Island is seeking a SNF Physician (MD/DO) to provide clinical oversight and leadership across skilled nursing facility settings. This role combines direct patient care coordination with supervision of Advanced Practice Providers.
Position Offers:
- Schedule: Monday–Friday
- Competitive compensation: $275,000–325,000 per year (DOE)

Key Responsibilities:
- Review and manage patient charts for quality and compliance in SNF settings
- Provide clinical supervision and support to midlevel providers
- Communicate regularly with skilled nursing facility leadership regarding patient care coordination
- Oversee clinical decision-making for geriatric patients in assigned facilities
- Support continuity and coordination of care across SNF settings
- Participate in team-based care planning and clinical oversight
Qualifications:
- Active, current Physician (MD/DO) licensure in New York, in good standing
- Minimum 1 year of experience in skilled nursing facility (SNF) settings; geriatric care experience preferred
- Strong communication skills and the ability to provide clinical supervision and oversight
