Full Time
Permanent Position
Seeking BC/BE Geriatric Medicine Physician (or IM/FM with geriatrics expertise) to join team in Attleboro, Massachusetts
Position Highlights:
Split schedule with 50% clinic and 50% community-based care
Coverage across Bristol and Norfolk Counties
Travel required for home and community visits
No or limited call responsibilities
Focus on complex geriatric population and chronic disease management
Collaborative interdisciplinary care model
Opportunity to incorporate telehealth and remote monitoring
Responsibilities:
Provide geriatric care in clinic and community settings
Manage complex medical conditions in older adults
Coordinate care with interdisciplinary team
Perform home visits and community-based evaluations
Develop and manage individualized care plans
Participate in care transitions and population health initiatives
Qualifications:
Board Eligible or Board Certified in Geriatric Medicine or Internal Medicine/Family Medicine with geriatrics experience
Active Massachusetts license and DEA or ability to obtain
Experience with complex geriatric patients preferred
Valid driver’s license and ability to travel required
