Overview

A rapidly growing outpatient primary care organization is seeking a Spanish-speaking Family Medicine Physician to join a collaborative clinic in Rockford, Illinois.

This practice operates under a value-based care model focused on delivering proactive and preventative care to Medicare-aged patients, emphasizing improved outcomes and stronger patient relationships.

Providers benefit from smaller patient panels, extended visit times, and robust clinical support.



Position Highlights

• Schedule: Monday – Friday
• Hours: 8:00 AM – 5:00 PM
• Setting: Outpatient primary care clinic
• Patient population: Primarily geriatric / Medicare
• Visit length: 30 minutes
• Panel size: capped around 450 patients per provider
• EMR: Epic



Clinical Support

Providers practice within a team-based care model including:

• Dedicated Medical Assistant
• Dedicated Scribe
• Shared RN support (1 RN per 2–3 providers)
• Collaborative physician and APP care teams

This structure allows physicians to focus on preventative medicine and chronic disease management rather than high patient volume.



Responsibilities

• Provide comprehensive primary care services
• Diagnose and manage acute and chronic medical conditions
• Perform preventative care and wellness visits
• Coordinate care with interdisciplinary clinical teams
• Document care using Epic EMR



Requirements

• MD or DO
• Board Certified or Board Eligible in Family Medicine
• Active Illinois medical license
• Active DEA license
Fluent or professional Spanish proficiency required

Experience with Medicare or geriatric patient populations preferred.

New graduates may be considered.



Compensation

Base salary typically ranges between:

$174,000 – $374,000+

Compensation varies based on experience, training, and geographic factors.
