PACE Program | Medical Leadership | Full-Time Weekday Schedule
365 Healthcare is seeking a Board-Certified Physician specializing in Internal Medicine or Family Medicine for a Locum Medical Director opportunity in Fayetteville, North Carolina. This role supports a Program of All-Inclusive Care for the Elderly (PACE), a care model designed to provide coordinated medical and supportive services that allow seniors with complex health needs to remain safely in their homes and communities.
This position combines clinical care and medical leadership, working closely with an interdisciplinary care team to coordinate and oversee care for elderly participants while ensuring quality outcomes and program compliance.
Schedule Model
• Monday – Friday
• Full-time day schedule
• Weekend call rotation as part of the provider schedule
• On-call rotation shared with Nurse Practitioners
Coverage Needed
• Full-time Medical Director coverage
• Ongoing locum assignment
• Part-time availability may also be considered
Program Overview
• Program of All-Inclusive Care for the Elderly (PACE)
• Patient population age 55+ requiring nursing-home level care
• Focus on helping seniors remain independent within their communities
• Multidisciplinary team-based care model
PACE programs deliver a comprehensive continuum of care that includes medical services, therapy, social support, and coordinated healthcare services designed to improve quality of life for elderly participants.
Clinical Responsibilities
• Conduct new participant history and physical examinations
• Perform periodic reassessments and ongoing clinical evaluations
• Develop and manage individualized treatment plans
• Provide medically necessary services across participant care settings
• Educate participants and families regarding chronic disease management
• Collaborate closely with nursing staff and interdisciplinary care teams
• Participate in care planning meetings and participant care conferences
Leadership Responsibilities
• Provide medical direction and clinical oversight for the PACE program
• Supervise Primary Care Providers and Advanced Practice Providers
• Support clinical outcomes and quality improvement initiatives
• Oversee Quality Assurance Performance Improvement (QAPI) program
• Develop and maintain medical policies, protocols, and standards of care
• Participate as a member of the leadership team to support operational goals

Qualifications
• Active North Carolina physician license
• Board Certified in Internal Medicine or Family Medicine
• Board Certification in Geriatrics preferred
• DEA / Controlled Substance registration
• Minimum one year experience caring for frail or elderly populations
• Previous supervisory or leadership experience preferred
• Clean malpractice and background history
• NPDB Self-Query report required
