LOCUM Medical Director – Geriatric Medicine – West Springfield, Massachusetts

PACE Program | Leadership Role | Full-Time Weekday Schedule

365 Healthcare is seeking a Board-Certified Physician specializing in Geriatric Medicine for a locum Medical Director opportunity supporting a PACE (Program of All-Inclusive Care for the Elderly) program in West Springfield, Massachusetts. This role combines clinical care with program leadership within a multidisciplinary model designed to provide comprehensive care to seniors with complex medical needs.

The PACE model focuses on helping seniors with chronic conditions receive coordinated medical and supportive services while remaining safely in their homes and communities whenever possible.

Schedule Model

• Monday – Friday
• Full-time schedule
• Participation in provider on-call rotation as needed

Coverage Needed

• Full-time Medical Director coverage
• Ongoing assignment supporting the PACE program
• Leadership and clinical oversight responsibilities

Program Overview

• Program of All-Inclusive Care for the Elderly (PACE)
• Patient population age 55+ requiring nursing-home level care
• Focus on maintaining independence and community-based care
• Multidisciplinary team care model

PACE programs coordinate medical, social, and supportive services to ensure seniors receive comprehensive care while maintaining their independence and quality of life.

Clinical Scope

• Conduct new participant history and physical examinations
• Perform periodic reassessments and clinical evaluations
• Develop and manage participant treatment plans
• Provide medically necessary care across care settings
• Collaborate with nursing and interdisciplinary care teams
• Educate participants and families regarding chronic disease management
• Participate in team conferences and care planning discussions

Leadership Responsibilities

• Provide clinical leadership and medical direction for the program
• Supervise Primary Care Providers and Advanced Practice Providers
• Oversee clinical outcomes and patient care quality
• Support Quality Assurance Performance Improvement (QAPI) initiatives
• Develop and maintain medical policies, protocols, and standards of care
• Participate as a member of the leadership team in operational planning

Qualifications

• Active Massachusetts physician license
• Board Certification in Internal Medicine or Family Medicine
• Board Certification in Geriatrics preferred
• DEA registration
• Supervisory or leadership experience
• Experience working with frail or elderly populations
• Clean malpractice and background history
• NPDB Self-Query report required
