We are seeking an experienced Geriatric Medicine Physician for a locum Medical Director opportunity within a PACE program in West Springfield, Massachusetts. This role focuses on providing comprehensive, community-based medical care for frail and elderly adults, emphasizing independence, dignity, and continuity of care.

The physician will serve in a clinical leadership role, overseeing medical services, guiding interdisciplinary care teams, and ensuring high-quality outcomes for older adults with chronic and complex medical needs. This is an excellent opportunity for physicians passionate about geriatric care, population health, and value-based medicine.

Job Responsibilities

• Provide comprehensive primary and geriatric medical care to PACE participants
• Conduct initial history and physical exams, periodic reassessments, and event-based evaluations
• Develop and manage individualized medical treatment plans
• Oversee and supervise Primary Care Providers and Advanced Practice Providers
• Provide medical leadership and clinical oversight within the interdisciplinary care team
• Participate in intake evaluations and eligibility assessments
• Collaborate in care planning, family conferences, and participant education
• Supervise chronic disease management and preventive care initiatives
• Participate in Quality Assurance and Performance Improvement (QAPI) activities
• Order and interpret diagnostic tests and laboratory studies
• Prescribe medications and coordinate specialty care as needed
• Participate in on-call rotation as required

Qualifications

• Board Certified in Internal Medicine or Family Medicine
• Board Certification in Geriatric Medicine preferred
• Active Massachusetts medical license
• DEA registration
• Minimum of one year experience caring for frail or elderly patient populations
• Prior supervisory or leadership experience required
• NPDB self-query within required timeframe
• Strong interest in community-based and interdisciplinary care models

• Full-time schedule
