Job description Job Title: Board Certified / Board Eligible Family Medicine or Internal Medicine – Primary Care Physician
Location: Bath, NY
Shift "Monday–Friday, 7:30 AM – 4:00 PM
Duration: Full time position

Job Summary
The Primary Care Physician will provide on-site outpatient primary care services across multiple healthcare facilities within the system. The physician will deliver comprehensive medical care in accordance with established clinical standards, policies, and procedures.
Place of Performance
Services will be provided on-site at one or more affiliated medical centers and outpatient clinics within the healthcare system. Specific locations will be assigned based on operational needs.

Qualifications & License
• Must hold a current, full, and unrestricted medical license in any U.S. state, territory, commonwealth, or the District of Columbia.
• Physicians with a history of restricted, suspended, revoked, surrendered, or denied licenses will not be considered.
• Board Certification
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Must maintain current BLS and ACLS certification (or equivalent).
• All continuing education requirements must be kept up to date.
• Certification documentation must be provided annually.

Job Duties and Responsibilities
Clinical Care
• Provide comprehensive primary care services within scope of practice and privileging.
• See approximately 12–13 patients per day, including acute, new, and routine visits.
• Perform histories and physical examinations.
• Provide ongoing outpatient and/or inpatient care for assigned patients.
• Carry out health promotion and disease prevention activities.

Documentation & Systems
• Complete all clinical documentation and close patient encounters by the end of each shift using the designated electronic medical record system.
• Order and monitor diagnostic testing and ensure timely communication of results:
• Results requiring action: within 7 calendar days
• Results not requiring action: within 14 calendar days
• Significant abnormalities must be communicated sooner, as clinically indicated.

Medication Management
• Prescribe, adjust, and monitor medications in accordance with established policies.
• Sample medications may not be provided to patients.
• Monitor and manage medication effects and side effects.
• Patient Communication & Education
• Communicate test results per established communication guidelines.
• Provide and document patient education and discharge instructions.
• Coordinate follow-up care with appropriate care settings.
• Compliance & Performance
• Be present and punctual for all scheduled clinics.
• Ensure patients are seen in a timely manner according to clinic access standards.
• Report any obstacles to meeting performance expectations through established channels.
