Job description

Job description • Medical Director role within a PACE program serving seniors with chronic care needs
• Provides comprehensive medical oversight and care coordination
• Supports seniors in maintaining independence while living safely in the community
• Reports administratively to the Executive Director
• Works within a collaborative, interdisciplinary care model

Job Responsibilities

• Provide general medical direction and supervision of participant care
• Oversee delivery of participant care and clinical outcomes
• Lead and oversee the Quality Assurance Performance Improvement (QAPI) program
• Develop, implement, and maintain medical policies, procedures, and clinical protocols
• Supervise and provide clinical leadership to PCPs and APPs
• Execute assignments within a collaborative Trinity Health PACE culture
• Establish and maintain operational and clinical goals as part of the leadership team
• Participate in provider on-call rotation as required

Qualifications

• Minimum of one (1) year documented experience working with frail or elderly populations – required
• Previous supervisory or leadership experience – required
• Prior experience in geriatrics – required
• Active, unrestricted Massachusetts medical license – required
• Board Certified in Internal Medicine or Family Medicine – required
• Board Certified in Geriatrics – preferred
• Experience or added qualifications in geriatrics – preferred
• DEA Registration – required
• CDS / Controlled Substance Certification – required

• Permanent, full-time position
• Leadership role
