Primary Care Physician – PACE Program
Locations: Central & Eastern Massachusetts (multiple sites available)
Compensation: $275,000–$300,000 annually, based on experience

Key Responsibilities

Provide primary, preventive, urgent, and chronic care to PACE participants

Conduct initial and biannual assessments and contribute to individualized care plans

Participate actively in interdisciplinary team (IDT) meetings

Coordinate specialty care and transitional care services

Deliver care across settings including clinic, home, and inpatient facilities

Participate in limited after-hours call rotation

Maintain timely and accurate clinical documentation
Qualifications

MD or DO licensed in Massachusetts; DEA required

Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred)

Minimum 1 year of experience with frail or elderly populations

Strong collaborative, communication, and clinical assessment skills

Valid driver’s license and reliable transportation required
