Job Responsibilities
• Develop personalized care plans centered around each member's individual goals
• Build and maintain meaningful, long-term relationships with your patient panel
• Offer same-day or next-day access and extended visit times for enhanced continuity of care
• Collaborate with outside specialists to ensure comprehensive, coordinated care
• Stay engaged with current clinical practices through ongoing development
• Practice independently in a relationship-driven, conflict-free setting
Qualifications
• Board certified or eligible in Family Medicine, Internal Medicine, or a related field
• Active medical license in Pennsylvania
• Experience in primary care with a strong focus on patient relationships
• Strong communication and interpersonal skills
• A passion for the old-school, relationship-centered approach to care
• Alignment with the mission and values of direct primary care
