Role Overview

• Conduct patient assessments, manage chronic and acute conditions, and adjust care plans
• Provide mentorship and clinical support to nurse practitioners and facility staff
• Ensure regulatory compliance and contribute to population health strategies
• Participate in the Medical Director Leadership Academy and shared savings programs
• Must reside within 30 minutes of Albany and work onsite 5 days per week
