Practice Setting

Qualifications
• Board Certified – REQUIRED
• BLS, ACLS, – REQUIRED
• Active MI – REQUIRED
• EMR, Heartland – REQUIRED
• Copy of the NPDB, self-query requires at presentation (Should be pulled within a month) - REQUIRED
Credentialing window: 60 DAYS (Temps can be issued in 1-2 weeks for CLEAN FILES.)
