- Schedule: Monday–Friday
- Status: Full-time
- Care Model: Community-based, interdisciplinary, participant-centered
🩺 Key Responsibilities
- Provide medical direction and oversight for participant care
- Conduct histories, physicals, and ongoing reassessments
- Develop and oversee individualized care plans
- Lead and support Primary Care Providers and Advanced Practice Providers
- Guide clinical quality initiatives and the QAPI program
- Participate in intake evaluations and care planning decisions
- Educate participants and families on chronic disease management and wellness
- Collaborate closely with nursing, therapy, social work, and care coordination teams
- Participate in on-call rotation as needed
🤝 Requirements :
- Board-Certified in Internal Medicine or Family Practice
- Board Certification in Geriatrics
- Active state medical license
- DEA registration
