Job description
Deliver medicine with purpose in a model designed for connection, compassion, and meaningful outcomes. As a Senior Care Physician, you will provide intentional, relationship-centered care to adults 65+ within a value-based model that prioritizes prevention, quality, and time with each patient. With small patient panels, expanded visit times, and strong clinical support, you will practice at the highest level while making a measurable difference in the lives of seniors who truly value their physician.
Position Highlights

Outpatient-only schedule, Monday through Friday, closed for lunch

Manageable daily volumes with 10 to 15 patients per day

Senior-focused care for Medicare and Medicare Advantage populations

Longer visits designed for preventive care and chronic condition management

Candidate Requirements

MD or DO, Board-Certified or Board-Eligible in Family Medicine, Internal Medicine, or Geriatrics

Interest in senior-focused, value-based, prevention-first care

Excellent communicator who thrives in a collaborative environment

Comfortable managing a 65+ patient panel with a relationship-driven approach

Bilingual (Spanish/English) preferred but not required
