Responsibilities and Duties
- Provide comprehensive outpatient care to geriatric patients
- Focus on chronic care management and preventative medicine to minimize hospital admissions
- Accurately code and chart patient encounters
- Utilize technology efficiently to support clinical workflows
- Maintain organized records and support continuity of care
Additional Information
- Active Florida medical license required
- Board certification in Family Medicine, Geriatric Medicine, or Internal Medicine required
- Basic Life Support (BLS) certification required
- 2-3 years of post-residency experience, including work with chronic care cases in community health centers
- Strong organizational and technology skills are essential
