Compensation: Base Salary $250K+ Incentive Bonus Signing Bonus up to $50K

We deliver coordinated, high-quality care to moderate- to low-income seniors with complex chronic conditions. With reduced patient panels and strong clinical support, youll have the time, tools, and autonomy to truly impact patient lives.
Role & Compensation
Base Salary: $250K+
Bonus: Up to 10% of base salary
Signing Bonus: Up to $50K
Partnership Track Available
Schedule: MondayFriday
Setting: 100% outpatient
Patient Volume: 1820 patients per day
Support Team: Dedicated MA, scheduler, and referral specialist
Patient Population: Seniors with 35 chronic conditions
Care Model: Value-Based Care
Requirements
Completed residency in Internal Medicine or Family Medicine
Board Certified/Board Eligible in FM, IM, or Geriatric Medicine
Bilingual (English/Spanish) required
