LOCUM – Medical Director (Geriatric Medicine, PACE Program)

365 Healthcare is seeking a LOCUM Physician – Medical Director for a full-time leadership and clinical position within a PACE (Program of All-Inclusive Care for the Elderly) model serving frail and elderly adults. This role combines medical oversight, interdisciplinary collaboration, and person-centered geriatric care.

Compensation

Role Overview

The Medical Director provides clinical oversight, ensures high-quality participant care, and leads medical components of the PACE program. Responsibilities include participant assessments, care planning, provider supervision, and advancing quality outcomes across the program.

Key Responsibilities

  • Perform histories, physicals, and periodic medical assessments

  • Develop individualized treatment plans

  • Participate in applicant evaluations for program eligibility

  • Communicate prognosis and care goals with participants and caregivers

  • Attend interdisciplinary team meetings and care conferences

  • Educate participants and families on chronic disease and wellness

  • Collaborate closely with nursing, therapy, and social work teams

  • Supervise PCPs and APPs

  • Order and interpret labs, diagnostics, and document appropriately

  • Participate in provider on-call rotation

  • Support quality improvement and operational goals

Required Clinical Expertise

  • Strong assessment and chronic disease management skills

  • Experience working with frail/elderly adult populations

  • Comfort working in a collaborative, interdisciplinary setting

  • Ability to manage dynamic participant needs

Candidate Requirements

  • Active Massachusetts state medical license — Required

  • Board Certified in Internal Medicine or Family Medicine — Required

  • Board Certification in Geriatrics — Preferred

  • DEA registration — Required

  • Prior supervisory experience — Required

  • Minimum 1 year geriatric or frail-elderly clinical experience — Required
