Job Title: Medical Director - Geriatric Medicine
Overview: An exciting opportunity awaits in West Springfield, MA, for a dedicated full-time Medical Director to lead a pioneering Program of All-Inclusive Care for the Elderly (PACE). This facility is committed to providing comprehensive medical oversight and quality care coordination for seniors, promoting independence and dignity in their communities. In this leadership role, you will be instrumental in ensuring that elderly patients receive the complete continuum of care, tailored to their unique needs.
Job Responsibilities:
- Conduct history and physical examinations on new participants and perform periodic reassessments.
- Develop medical treatment plans and provide necessary services across various locations.
- Interview and evaluate applicants for the PACE Organization and their caregivers.
- Discuss medical conditions and future care planning with participants and their caregivers.
- Collaborate in creating individualized nursing treatment programs for participants.
- Participate as an active member of the interdisciplinary PACE team, providing expertise on diagnoses and treatment options.
- Educate participants and families about health maintenance and chronic conditions.
- Oversee the healthcare and treatment programs developed by the interdisciplinary team.
- Perform physical examinations and order relevant laboratory tests.
- Participate in provider on-call rotation as required.
Qualifications:
- Current and active plenary physician license to practice in Massachusetts.
- Board Certification in Internal Medicine or Family Practice required.
- Preferred Board Certification in Geriatrics.
- Previous supervisory experience in a medical setting.
- Minimum of one (1) year of experience working with a frail or elderly population.
- Effective skills in physical assessment and chronic disease management.
- Desire to work in a creative alternative healthcare setting.
- Recent NPDB Self Query Report (within 30 days active).
- Able to manage changing priorities based on participant needs.
License and Certification Requirements:
- Current and active plenary physician license in Massachusetts - [REQUIRED]
- CDS Certification and DEA Registration - [REQUIRED]
- Board Certification in Internal Medicine or Family Practice - [REQUIRED]
- Board Certification in Geriatrics - [PREFERRED]
- Experience or additional qualifications in geriatrics - [PREFERRED]
- Previous supervisory experience - [REQUIRED]
