Job description
Seeking a full-time Geriatric Medicine Physician to provide medical oversight, primary care, assessments, and leadership within a PACE program serving frail and elderly adults.
Job Responsibilities
Evaluate new applicants and participate in Intake & Assessment decisions.
Discuss conditions, prognosis, and care goals with participants and caregivers.
Participate in interdisciplinary team meetings and provide clinical guidance.
Oversee chronic disease management and collaborate with nursing.
Supervise APPs and PCPs; support QAPI and policy development.
Perform physical exams, order diagnostics, and document results.
Participate in provider on-call rotation as needed.
Qualifications
Board Certified in Internal Medicine or Family Medicine (Geriatrics preferred).
DEA required; CSR/CDS required.
Minimum 1 year of experience with frail or elderly patients.
Prior supervisory experience required.
Experience in geriatrics preferred.
