ThedaCare, Wisconsin's largest non-profit healthcare system, is seeking a Senior Care Physician to join our established team in Appleton. You will be responsible for working with PALTC patients in community living facilities, developing relationships while administering high-quality patient care.
Position Details:
- Full-time (4-10s) or Part-time options available with flexible schedules!
- Location: Appleton
- The team consists of 5 Physicians and 8 APCs
- 2-3 SNFs a day (patient schedule built within EPIC)
- On-Call by phone only: 1:11
- Patient census: 8-12
- Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
- 27 Facilities total between skilled nursing and assisted living facilities
- Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve
Job Responsibilities
*Acute visits, compliance visits, & discharge visits for short and long-term residents
*Advance Care Planning & Goals of Care discussions
*Minor procedures at the bedside
