Opportunity:
A leading integrated health system in central Wisconsin is seeking a BC/BE Geriatrician, Family Medicine, or Internal Medicine physician to join its collaborative geriatrics team serving Berlin and Oshkosh. Providers enjoy flexibility, strong administrative support, and the opportunity to build lasting relationships with patients across post-acute care settings.

#### Position Highlights:
Schedule Options: Full-time or part-time, with flexible hours
Team Structure: 5 Physicians and 8 Advanced Practice Clinicians
Patient Load: 8–12 patients per day, typically across 2–3 facilities
Facilities: 27 total across SNF and ANFL settings
Call: Phone-only, 1:11 rotation
Support Staff: RNs and schedulers coordinate visits, triage calls, and assist with logistics
Growth Opportunities: Potential for Medical Director roles in partner nursing facilities

#### Candidate Requirements:
Board Certified / Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
Eligible for or active Wisconsin Medical License
Strong interest in geriatric and post-acute care medicine
Excellent communication, collaboration, and patient engagement skills

Guaranteed Base Salary plus Value-Based Incentives
Signing Bonus available
Paid Time Off: 35 days annually
CME: $4,500 per year
