Primary Care Physician – Value-Based Outpatient Care

Compensation:

  • Base Salary: $215,000 – $245,000 (commensurate with experience and location)

  • Incentive Bonus: Based on quality metrics and patient outcomes

About the Role

We are seeking an experienced and compassionate Primary Care Physician (PCP) to join our expanding network of innovative neighborhood clinics. Our model emphasizes value-based, patient-centered healthcare—focusing on prevention, wellness, and better outcomes for local Medicare populations.

Our physicians are supported by large, integrated care teams, enabling them to deliver quality care over quantity of visits. You’ll practice medicine the way it’s meant to be—personal, proactive, and rewarding.

Role Details

  • Support Team: Scribe, Medical Assistant, and Social Worker

  • On-Site Resources: In-house lab with phlebotomist and access to RubiconMD for specialist consultations

  • Model: Value-Based Primary Care

  • Patient Transportation: Complimentary van pickup within 5 miles of the clinic

  • Community Engagement: Wellness activities including yoga, exercise, dance, and health events

  • Clinic Composition: Average of 3–5 providers per location (mix of Physicians and NPs)

Job Responsibilities

- Provide comprehensive, preventive, and chronic care management for Medicare-aged patients.
- Collaborate with a multidisciplinary care team, including scribes, medical assistants, and social workers.
- Utilize evidence-based practices to drive better health outcomes and reduce hospital admissions.
- Maintain accurate, timely documentation within Canopy and Greenway EMR systems.
- Engage with patients through wellness initiatives and community-based programs.

Qualifications

- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.)
- Board Certified in Internal Medicine or Family Medicine
- Active, unrestricted, non-probationary state medical license
- Active DEA license
- Minimum of 2 years of clinical experience (no current residents)
- Bilingual (Spanish) preferred
- Fellowship training in Geriatrics preferred

Working Hours

- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- Patient Volume: 13–16 patients per day
- Visit Length: 20 minutes for routine visits; 40 minutes for diagnostic/new patients
