Compensation:

  • Base Salary: $215,000 – $245,000 (commensurate with experience and location)

We are seeking a Primary Care Physician to join our growing network of innovative, neighborhood-based primary care centers serving local Medicare patient populations. Role Details

  • Clinic Support: On-site lab and phlebotomist, access to RubiconMD specialist consultations

  • Transportation: Complimentary van service for patients within 5 miles

  • Team Composition: 3–5 providers per clinic (mix of NPs and Physicians)

  • Community Engagement: Wellness classes, events, yoga, dance, and health-focused activities

Job Responsibilities

- Provide comprehensive, preventive, and ongoing primary care to Medicare patients.
- Work collaboratively with a multidisciplinary care team including a scribe, medical assistant, and social worker.
- Deliver care aligned with value-based health models, focusing on quality outcomes and reduced hospital admissions.
- Maintain accurate and timely documentation in Canopy and Greenway EMR systems.
- Participate in community engagement and wellness programs, including patient education and clinic-based activities.

Qualifications

- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Minimum of 2 years of post-residency clinical experience (no current residents)
- Board Certified in Internal Medicine or Family Medicine
- Active, unrestricted state medical license and DEA registration
- Bilingual (Spanish) preferred
- Fellowship training in Geriatrics highly desirable

Working Hours

- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Patient Volume: 13–16 patients per day
- Visit Duration: 20-minute routine visits; 40-minute new or complex visits
