Geriatric Primary Care delivers innovative, individualized healthcare for seniors in skilled nursing and long-term care (LTC) settings. We’re seeking a Part-Time Medical Director to lead care across partnered facilities. You’ll provide hands-on patient care, mentor Advanced Practice Providers (APPs), and collaborate with facility leadership to elevate outcomes and experience.

We’re seeking a Part-Time Medical Director to lead care across partnered facilities. You’ll provide hands-on patient care, mentor Advanced Practice Providers (APPs), and collaborate with facility leadership to elevate outcomes and experience.
Job Responsibilities
Clinical leadership & supervision of APPs (NP/PA)
Facility collaboration: partner with administrators and key stakeholders
Quality initiatives: participate in QAPI/quality improvement and ensure CMS compliance
Program development: help shape and expand services in the region
Documentation & EMR best practices; timely charting and coordination of care
Qualifications
Active, unrestricted medical license (or eligibility) for the practice location
Experience or strong interest in geriatric medicine and post-acute care
Eligible for credentialing at affiliated facilities
Working Hours
No call requirements
Opportunity to expand hours as desired
Skills
Clear, empathetic communication with patients, families, and facility teams
Collaborative leadership and APP mentorship
Comfort with EMR, documentation, and value-based/quality metrics
