As a Primary Care Physician, you will provide high-quality, outpatient care in a value-based care model, focusing on building long-term relationships with patients while driving positive health outcomes.

Practice Highlights:

  • 100% outpatient primary care

  • Patient population: seniors with 3–5 chronic/complex conditions (e.g., heart disease, diabetes, hypertension)

  • Average of 10–12 patients per day

  • Patient panel capped at ~650

  • Dedicated care team including MA, scheduler, referral specialist, and full leadership support

Job Responsibilities

- Provide comprehensive primary care to adult patients in an outpatient setting
- Perform minor procedures, including suturing, joint injections, skin biopsies, and wound care
- Collaborate with an integrated care team to deliver patient-centered care
- Maintain accurate and timely medical records

Qualifications

- M.D. or D.O. degree (required)
- Board Certified/Board Eligible in Family Medicine or Internal Medicine (required)
- Current, unrestricted medical license (required)
- Active DEA registration (required)
- Fellowship in Geriatrics (preferred, not required)

Working Hours

- Monday–Friday, 8:00 a.m.–5:00 p.m.
- Shared call schedule (e.g., 1:32 for 32 providers in market)

Skills

- Proficiency in basic office procedures: suturing, joint injections, skin biopsies, and wound care (required)
