The Primary Care Physician (PCP) serves as a clinical leader in a team-based, value-driven care environment focused on delivering high-quality care to seniors. Supported by a multidisciplinary team—including Advanced Practice Providers, PharmDs, Nurses, Behavioral Health professionals, and more—the PCP is empowered to spend more time with fewer patients, offering a concierge-style experience that improves outcomes through coordinated care, data-driven tools, and strong community relationships.

Job Responsibilities

• Provide outpatient care focused on continuity, prevention, and disease management.
• Maintain accurate medical histories and records; refer patients to specialists as needed.
• Evaluate and treat patients according to established standards of care.
• Monitor quality of care using data and chart reviews.
• Coordinate patient services including referrals, hospital/SNF transitions, DME, and home health.
• Participate in daily care team huddles and serve as a key medical resource.
• Collaborate with leadership to foster a cooperative, professional practice environment.
• Complete documentation timely and work with coders to optimize coding specificity.
• Follow clinical policies and protocols; meet regularly with leadership to review outcomes.
• Support growth opportunities for new or existing services.
• Participate in local on-call programs as needed.
• Ensure compliance with licensing, certification, and accreditation requirements.
• Spend 100% of time focused on direct patient care, including administrative tasks related to care.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Current and unrestricted medical license (or willingness to obtain) in state of practice.
• Strong verbal and written communication skills.
• Excellent interpersonal skills and ability to work collaboratively in a team-based model.
• Commitment to value-based care and continuous learning.
• High level of patient service and professionalism.
• Basic computer proficiency, including EMR and email.
• Willingness to participate in TB screening as part of a patient-facing role.

Working Hours

Monday - Friday, 8:00a-5:00p

Skills

Value Based Care Experience, or outpatient primary care experience is preferred.
