Primary Care Physician – PACE Program (Geriatric Population)
Schedule: Monday – Friday, 8:00 AM – 5:30 PM
On-Call: Rotating, telephonic only (approximately one week at a time)
Overview:
We are seeking a compassionate and team-oriented Primary Care Physician to join a well-established Program of All-Inclusive Care for the Elderly (PACE). This role is focused on delivering coordinated, comprehensive, and patient-centered care to a frail and elderly population. The physician will work collaboratively within an interdisciplinary team to support the ongoing health and quality of life of each participant.
Key Responsibilities:

Conduct comprehensive initial, semi-annual, annual, and periodic assessments of participants.

Participate in the development and review of individualized care plans in collaboration with the interdisciplinary team (IDT).

Review and sign care plans at least every six months.

Provide primary care, chronic disease management, and acute care to participants.

Supervise and collaborate with nurse practitioners and physician assistants as needed.

Address new medical issues between formal assessments and update care plans accordingly.

Actively participate in IDT meetings, care conferences, and family discussions as needed.

Order and review diagnostic tests and prescribe medications.

Coordinate specialist referrals and follow-up care in line with participant goals and values.

Provide after-hours, phone-based on-call medical coverage on a rotating basis.

Coordinate care with hospitalists during participant admissions and support discharge planning.

Manage care for participants residing in nursing homes, as assigned.

Maintain timely and accurate documentation using eClinicalWorks (EMR).

Participate in clinic meetings, task forces, ethics discussions, infection control initiatives, and quality improvement efforts.

Ensure participant records are accurate and up to date, including care plans, problem lists, flow sheets, and progress notes.

Work closely with clinic and administrative staff to maintain smooth clinic operations.

Uphold confidentiality standards in compliance with HIPAA.

Comply with all policies, procedures, training requirements, and safety protocols.
Requirements:

M.D. or D.O. with current, unrestricted California medical license.

Board certification or eligibility in Internal Medicine or Family Medicine.

Experience working with geriatric or frail populations (PACE experience preferred).

Current DEA license (or willingness to obtain).

Certification in Basic Life Support (BLS).

Strong commitment to interdisciplinary, patient-centered care.
