Job Overview – Family Medicine Physician (Outpatient):
Compensation: $250,000 – $275,000/year
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM (with rotating Saturdays)
Join our client’s community health team as a Family Medicine Physician (Outpatient) in Brooklyn, NY, providing full-scope primary care to patients of all ages in a diverse, underserved setting. In this outpatient role, you’ll conduct comprehensive evaluations, develop treatment plans, coordinate specialty referrals, and deliver preventive care to pediatric, adolescent, and adult populations. Ideal candidates are board-certified physicians committed to health equity, social determinants of health (SDOH), and advancing care in community-based environments.

Responsibilities as the Family Medicine Physician (Outpatient):
- Primary Care Delivery: Provide comprehensive care for patients of all ages, including exams, diagnostics, and treatment planning.
- Preventive Health: Administer routine screenings, immunizations, and patient education based on individual needs.
- Care Coordination: Collaborate with specialists, behavioral health, and support teams to ensure integrated care.
- Diagnostics & Treatment: Order and interpret labs and imaging, develop diagnoses, and initiate medical interventions.
- Documentation & Quality: Maintain accurate EMR records and meet compliance, productivity, and quality standards.
- Community-Based Care: Deliver culturally sensitive care while addressing challenges related to social determinants of health.
- Education: Medical degree (MD or DO) from an accredited institution is required.
- Licensure: Active and unrestricted New York State medical license is required.
- Certification: Board Certification in Family Medicine is required.
- Experience: Prior outpatient experience preferred, especially within underserved or community health settings.
- Technical Skills: Proficient in electronic medical records (EMR), with experience in eClinicalWorks strongly preferred.
- Skills & Attributes: Compassionate and clinically skilled in caring for pediatric and adult patients, with strong communication abilities and a commitment to underserved communities.
